1. Using a distal transparent cap on a standard gastroscope is an excellent technique for flattening duodenal folds, dramatically improving the visualization of an elusive or hidden major papilla.
2. The cap allows the endoscopist to gently manipulate the periampullary tissue, providing a stable, head-on view of the papilla that is often critical for difficult cannulations or identifying subtle ampullary adenomas.
3. When standard forward-viewing or side-viewing scopes fail to provide adequate exposure of the papilla due to severe angulation or redundant folds, deploying a transparent cap is a simple, cost-effective rescue strategy.
